Here at Seacoast Dental Associates, we want every visit you make to our clinic to be efficient and worry-free. Please arrive at your first appointment 15 minutes early with your insurance card readily available so that you can take your time filling out the patient registration forms. The forms are also readily available to download online if you would prefer filling them out ahead of time and bringing them along with you.
Complete the New Patient Forms online
At your first visit (for adults), we will conduct a 90-minute exam, which includes the following:
- New patient exam
- Complete dental charting
- A review of your health history
- Radiographs when necessary
- Digital Imaging
- Hygiene evaluation
We will go over the results of your exam and work closely with you to develop a dental plan that is tailored to your specific oral health needs.
For children, the first visit will consist of time for our dentist and your child to become acquainted, do a thorough exam, and clean your child’s teeth– as long as your youngster is comfortable.
Please note: If you need to reschedule your appointment for any reason at all, we are happy to do so with 24 hours’ notice prior to your appointment.
